Leprosy Mailing List – December 6, 2019
Ref.: (LML) ENL and pregnancy
From: Ramesh Sharma, Pokhara, Nepal
Dear Pieter,
I would like to share a case scenario and seek advice from the experts.
We have a young female, gravida 2, para 1. who is in her 3rd month of period of gestation. She has developed recurrent and severe ENLs. When she was diagnosed leprosy, she had already conceived which came to the knowledge only after a few days of start of the treatment. It looks like she is going to have multiple such episodes of flaring up of ENLs during the period of her pregnancy. Looks like she needs thalidomide.
What would be the best option for us?
- To terminate the pregnancy and start thalidomide or
- to continue the pregnancy and give prednisolone?
The patient and the patient party have complete faith in our decision and have left it up to us to decide and have agreed to follow our advice.
Looking forward to your suggestion.
Regards,
Ramesh,
