Dear Dr. Duncan,

 

Thanks for your comments about the management of leprosy reactions in pregnancy [LML Nov. 17th, 2012]!

 

When about everything fails and there is remaining significant peripheral nerve function loss, one can consider a nerve decompression.  In general it can be done under local anaesthesia / plexus block and when done properly, it can strongly reduce nerve pain at the site of entrapment while nerve function can be restored when performed at an early stage.

Many leprologists are hesitant to explore this option but the procedure is not much different from the routine carpal tunnel release as performed on a daily basis in any hand surgery practice.
