Dear Pieter,
I would like to refer to the letter of Tarun Narang (LML, June 16, 2015) and thank him for addressing the issue of PN leprosy with its dilemma's.
The diagnosis can often be confirmed by a "cytological needle aspiration" of an enlarged nerve, a simple and safe technique. For those interested please see the article on this with the same title.
In general the treatment is MDT as for multi-bacillary leprosy, and indeed in parallel, there is a risk of ongoing nerve function loss due to reaction/ silent neuritis.
Where corticosteroids fail a nerve decompression can be considered, this with often good results.
The one that can design neuro-protective agents effective in leprosy should be awarded the Nobel prize for medicine??!
