Dear Pieter,

 

We eliminated the expertise to diagnosis leprosy:

 

-       Almost no more efforts are being done to teach the professionals in the field. Since the ILEP members are not working in the field, all countries stopped to make on service training.


Fortunately, in Brazil, in a few areas only, we have the NGO Alliance Against Leprosy (AAL), and thousands of patients are still being diagnosed.


-       The laboratory people do not know how to collect and stain Slit Skin Smears and most pathologists just do not know what is Faraco, doing only common Ziehl, a staining which destroy any possibility to find M.leprae in tissues.

-       Diagnosis is only in reference centers, after several years, and household contacts are not evaluated and, of course, followed. 

-       Dermatologists do not know how to do the palpation of nerves, and most thinks that leprosy is a disease of the skin, like vitiligo or urticaria.

 

Ignorance is the rule: 

-       One leproma, PB leprosy. Six lepromas, MB leprosy.

-       Several affected nerves and no skin lesion, no leprosy.

-       Diffuse leprosy does not exist, once "it has to have patches".

 

It is the end of the world.
