Dear Pieter,
Thalidomide is an established teratogen and is banned in pregnant women. I feel that there is no way around this caveat, hence, my belief is that it is unethical to give it to women who are not on double or triple contraceptives and/or who can not be relied upon to abstain.
There are reported newer biologicals that are anti tumor necrosis factor alpha, and appear safer than thalidomide to use in pregnancy. I have no experience with these except in psoriasis. I also think they are very expensive but may be obtained for compassionate use.
