Ref.: (LML) Higher risk of relapses in BL/LL with BI 4 or more after MDT-U and MDT 12 doses

Dear Pieter,
I am referring to the submission by Dr. Jaison Barreto regarding relapses after MDT-U and 12 months MDT. While the concerns about the likely increase in relapse rates after MDT-U are genuine, I doubt if an official communication has come from somewhere recommending its implementation in routine programmes. This problem would be compounded by the challenges associated with follow up (any not just long-term) of patients after MDT in routine control programme settings. What would be his recommendation for follow up patients after MDT?
It would be good to provide LML readers with definite references to documents from which the statements are derived :on diagnosis of leprosy (no patches = no leprosy), classification of patients with a single lesion (even if it would be a leproma) as well as the classification and treatment of patients with several neural lesions. This evidence would enable readers to respond to the statement that "diagnosis and/or treatment schemes are determined politically".
Guidance is also required on the definition of "Multidrug resistant leprosy"; would there be something similar to the definition of Multidrug resistant Tuberculosis?
