Dear Pieter

 

 

We have a 40-year female patient with borderline tuberculoid leprosy with type 1 reaction under 3rd month of MB MDT. She developed cushingoid features with oral prednisolone and when the prednisolone was tapered bit fast, the reactions flared up. We then started clofazimine in a dose of 100 mg TDS few days back since I learned of it from a colleague. 

 

1. What could be the options in type 1 reaction when patients develop toxicity or severe adverse effects with steroid?

2. How effective or what is the evidence of clofazimine in type 1 reaction?

 

I work in a leprosy centre called Greenpastures Hospital, Pokhara, Nepal.
