Dear Pieter and colleagues,
Has the final stage of elimination of leprosy come, or should we be concerned about a persisting high incidence in some geographical areas? Are we fully aware of potential alternative transmission routes? Beside armadillo's, red squirrels have been shown to be a reservoir of leprosy causing pathogens, and likewise there may be other reservoirs. What is the impact of these reservoirs in maintenance of transmission? Additional factors may play a role, e.g. what is there a role for vectors and for the environment?
According to one of the reviewers, our paper is the most thorough review of environmental sources of leprosy bacilli in the last 40 years and includes more than 75 years of leprosy research. The review shows a shift towards molecular techniques in studies aimed at tracing and characterization of the causative agents of leprosy, and elucidation of pathogenesis. It also shows that overall, the great variety of studies lacks standardization and hampers comparative evaluation.

One Health
Factors and mechanisms involved in maintenance of leprosy transgress the discipline of human healthcare. For that reason, we advocate a One Health transdisciplinary research approach, integrating human, animal, and environmental health disciplines.
