Dear Pieter & colleagues,

Volunteers in India came together to produce a reasonably good mapping and monitoring real time system to track COVID 19 reports.

This was for COVID, not HD. Yet, it suggests the importance of attracting and empowering top local talent. Not every bright young person in endemic countries will choose to devote all their time to fighting HD. However, if they contribute even part of their time, it can help transform outcomes.

Pre-cooked measures imposed on endemic countries tend to have disappointing outcomes and impact. That is because they sideline an important ingredient in highly effective interventions: the skill and devotion of highly talented front line staff and volunteers who are keen to improve things.

In the old days endemic countries were regarded as the limbs and affluent countries as the brains. Now it is commonplace to see people from endemic countries as the CEOs of large corporations (eg Microsoft, Google/Alphabet, Adobe etc). The world of business is interested in financial outcomes, not in maintaining old preconceptions. In HD we are interested in health outcomes.

A conscious effort to attract and finance top talent in endemic countries is likely to help improve outcomes and impact in HD.

Best,

Joel Almeida
