Dear Pieter,

I'm writing about the ILEP Learning Guides, written in 2000 or 2001, with titles such as "How to diagnose and treat leprosy", "How to recognise and manage leprosy reactions", etc.  They are still freely available online at <https://www.ilepfederation.org/about-leprosy/publications/>

Users are also free to print hard copies, if these are needed.  Originally, there were CDs with HD graphics for printing, but these are no longer available.

I have two questions for LML readers:

1. Does anyone still have the old HD CDs from which reprints can be made?  Some programs would like to have access to these, and I'm sure the files would be easy to transfer now, even though they were too big to email in 2001!

2. Do other programs still use the printed versions of the ILEP Learning Guides, and if so, is there interest having higher quality graphics made available, to improve the quality of the printing, perhaps with some editing of the text also?

Dear Pieter,

 

An interesting discussion in LML to differentiate between diabetic neuropathy and pure neurotic leprosy neuropathy.

 

I would like to say that both diabetes and leprosy without complication need simple instructions like self care (put feet in the water, clean, rub with vegetable/ coconut or hand body for overcoming dry skin/cracks). But also, simple exercises to improve blood circulation, strengthen muscles and to keep joints mobile to prevent complications. In some places both types of patients take part in Self Care Groups. In attached file find feet exercise video (in Bahasa Indonesia); it may be useful.

Dear Pieter,

 

Interesting ongoing conversation. I have been decompressing nerves as a student of doctor Dellon for the past 16 years in Ecuador and in the USA. I run a surgical team that performs peripheral nerve surgery on upper and lower extremity nerves in Hansen's patients in South America. Our results have been excellent in long-term follow-up for return of motor function, decreased limb pain and increased sensation.

 

The Hansen nerves do not have amyloid deposition such as what we see in the diabetic patient however they are quite swollen and show obvious signs of impingement at the fibro-osseous tunnels at the joint levels where these nerves are decompressed in both upper and lower extremities.

 

Interestingly we have had some patients with diabetes and Hansen's Disease which creates a real double crush type syndrome on the nerve.

In his LML message of April 22, 2018, Eric Slim stated that "Nerve enlargement can also be seen in diabetes related neuropathy but structural differences between diabetes and leprosy nerve impairment can be seen on ultrasound".  He also writes when Salvatore Noto questioned him about this statement: "that I was referring to the work of dr Dellon, who's surgical treatment consists of decompression of the nerve, based on the hypothesis of increased water content in the nerve resulting in subperineurial and endoneurial edema. An example for this hypothesis is papilopathy of the eye, occuring in diabetes, also caused by axonal edema."

 

Differential diagnosis of enlarged peripheral nerve (by Michael Donaghy Department of Clinical Neurology, University of Oxford, Radcliffe Infirmary, Oxford OX2 6HE, UK. Practical Neurology, 2003, 3, 40–45):
- Leprosy,
- Hereditary motor and sensory neuropathy,
- Neurofibromatosis,
- Refsum's disease,
- Perineuroma/localized hypertrophic neuropathy,
- Nerve tumours
- Amyloidosis

Diabetes is not mentioned.

 

When asking Wim Theuvenet about Eric"s statement, he writes: "I agree with Dr. Eric Slim that also in diabetes the nerves are often found to be enlarged, this in 30 to 60 % of the cases and depending on the screening technique used. This is the basis for the nerve decompressions described in the Ph.D. thesis (Utrecht March 6, 2007 Surgical approach to lower extremity nerve decompression in the patient with diabetic neuropathy) of Prof Lee Dellon. In general, the swelling is more diffuse and less tender than in leprosy. Indeed the D.D. on the basis of nerve enlargement can be difficult!

 

In the first place they do not refer to (peripheral nerves of) the hand/arm. In the second place they do not mention hardening of the nerves. In the third place are these edematous nerves palpable enlarged or only as seen by ultrasound? In the fourth place are the lateral popliteal and sural nerves involved in diabetes?
