Leprosy Mailing List – November 19, 2015
Ref.: (LML) Draft WHO Leprosy Strategy 2016-2020
From: Salvatoro Noto, Bergamo, Italy
Dear Pieter,
Thank you very much for your efforts to run the LML. I would like commenting about the treatment of different forms leprosy. In attachment are 6 slides of 2 patients of mine.
Slides 1-5 relate to a patient near the lepromatous end of the leprosy spectrum. His morphological index (MI) was positive after several months of treatment with 3 antileprosy drugs, kindly see slide 1. The immune system of patients with very high bacillary load has almost no capability of fighting M. leprae. Their treatment is longer.
Slide 6 is about a leprosy patient near the tuberculoid end of the spectrum, with very few lesions on his elbow only and, negative skin smear. After a few months of treatment with 2 antileprosy drugs his lesions disappeared. The immune system of patients with very few bacilli has not lost his capability of fighting M. leprae and shorter treatments will be enough.
Patients have to be treated according to the manifestations of their disease; leprosy is no exception!
Yours sincerely,
Salvatore
