Dear Colleagues,
The International Leprosy Congress in Beijing is coming near and so the session on: “Clinical problems in leprosy”. This session will be held in room 201 CD, in the afternoon of September 18th, from 13.30-15.00 hours.
The objective is to discuss clinical problems: it is purely a session to exchange ideas and experiences, and to try to answer each other questions. Everybody is invited to attend. It is hoped for, and expected, that the cumulative knowledge will be able to solve most problems. Answers may not always be evidence based, but will be backed by experience.
The discussions will be led by Professor Rao PN and me, but there will be no speaker or group of experts to answer the questions. Professor Karr HK has offered to attend and share his experience as many other LML readers did. All of us are the “experts”.
By this LML message we invite not only the clinicians but also the researchers. Because their knowledge is needed to give an answer to the clinical questions and maybe we can give an answer to their questions.
Let us make a success of the congress and this session!
Some questions raised so far:
- How to diagnose leprosy, do we need to revise the cardinal signs?
- Is there an alternative for counting of the lesions? Do we include involved nerves?
- How do we teach people to differentiate leprosy from other diseases?
- What do we do with skin smears, can it be done at primary level? Are there alternatives?
- When does leprosy have to be treated or retreated?
- How to differentiate relapse from reaction?
- How treat, evidence based, a reaction in children?
- How to detect silent nerve damage?
- What is happening during silent nerve damage?
- How to treat highly multibacillary patients with impaired liver function?
- Are there clinical predictive symptoms for the risk of new or further nerve damage?
- When do we do nerve surgery?
There are most likely more questions. Ask them at the meeting (or send beforehand to LML)!
I would like to quote Grace Warren: “Where one does not think about, one never sees”. “What one does not recognize one will never diagnose”.
A summary of the meeting will be published in LML.
