I would like to add to the comments from Cairns Smiths and Grace Warren (LML of 13 and 24 January 2017).
I was once introduced about mono filament test (MFN) in leprosy training ALERT Ethiopia in 1999. However, Mono filaments were not available in the field. In Indonesia we used ball point pen for hand and feet assessment (Nerve function Test). I believe, that by more practice, more sensitive we are. Sometime patients have thick skin hand and feet might be difficult to do and need more pressure. Similar to test with cotton for patches need more practice, patient and genteelly. Those tests with pen for hand and feet and cotton for patches look easy but again need practice/skill.
If we have good communication to patients, ask them how he/she feels about the skin, feet or hand. He/she will tell you if she/he feels/experience differently or there is different now compare to the past or there is different between right and left side .
