Dear Dr Noto,
I refer to prof. Coster’s message (of yesterday). I won't comment on the need for additional therapy pre-renal transplant as I'd be asking the same questions myself, but as a Transplant Infectious Diseases section director, I'd also suggest the patient receive dapsone for post-transplant suppression. The drug can serve 2 purposes-- 1) Pneumocycstis prophylaxis; and 2) secondary suppression assuming the primary LL/ENL is adequately treated pre-transplant.
