Dear Dr. Noto,
I strongly agree that there is a relation between stress and ENL reaction.  We have also seen many patients readmitted after few days of discharge from hospital. 
There are patients who got worse at home even with the same dose of steroids they were getting at hospital.  We have a leprosy affected person working in our hospital, who used to have recurrent ENL reaction.  His reaction was over after he got a job in our hospital about 7 years ago.  He recently married and has a child now.  There are other leprosy affected people who got better after getting a job somewhere else.  So we should think about this and would be great if we could do research on this matter.
