I read the question of Wim Theuvenet (LML, October 13, 2019). This is very valid because many people in leprosy are getting incorrect information.
Concerning biopsies after RFT many times viable bacilli are found. A good one is from long ago: Lepr Rev (1997) 68, 131 – 138. Higher incidence of viable Mycobacterium leprae within the nerve as compared to skin among multibacillary leprosy patients released from multidrug therapy. V. P. S HE TTY, K. S UC H I T R A, M. W. Concerning nose smears these are unreliable because the nose is like a vacuum cleaner. In an endemic area it is easy to sniff M. leprae dead or alive. PCR and Nasba or Reverse transcription polymerase chain reaction (RT-PCR) may then be transient positive.
And to make it more difficult even PCR detection of M. leprae targeting 16S ribosomal RNA is for a few months positive after the real dead of M. leprae.
