Dear Dr. Schreuder,

 

If patient is HD LL with high B.I. and unable to take the regular MB -MDT I use alternative meds: Rifampicin 600 mgs, Ofloxacin 400 mgs and Minocycline 100 mgs taken together once a month preferably at bedtime (some patients have mild vertigo with Mino) until negative smears.  This is usually after 18 to 24 months. These are all bactericidal agents.  Generation time for M. leprae is long (about 2 weeks) but every 2 weeks of Rifampicin may cause flu like symptoms in some patients.  Once a month is very tolerable just make sure patient has no Tuberculosis.
