Dear Pieter.


This note is in reference  to Salvatore's communication concerning the treatment of different forms of leprosy LML, November 19, 2015). His view is quote, "Patients have to be treated according to the manifestations of their disease; leprosy is no exception!" 


I think every one of us agrees to this statement. In another word, Salvatore is suggesting evidence based medicine or personalised medicine. This is more so in case of leprosy particularly at the time of declining incidence but continued transmission.

 

I do not follow why in endemic regions, clinicians and the available lab facilities are not adopting to monitor patients using few of well-established biomarkers to ensure that treatment per individual patient is completed before releasing them routinely.


I feel that leprologists will continue in picturing leprosy as very special public health problem instead of integrating this disease within the health system.
