Dear Pieter,

 

I would like to refer to the letter from Ben Naafs (LML, June 10, 2018) and the letter written by Diana Lockwood and colleagues in PLoS NTDs.

 

About chemoprophylaxis, I would like to know what is its value, once less than 20% of household contacts are effectively evaluated?

 

Also, most of bacilloscopic evaluation, both from histologic sections and slit skin smears, are done under bad conditions. Almost all reference histopathology laboratory's countries I have visited do not know Faraco-Fite for histologic staining of M.leprae, as well as the microbiolgy labs do not know that M.leprae and M.tuberculosis have extremely different acid fast resistance to destaining. 

 

First we need to teach again how to confirm the diagnosis, clinically and by Lab assays. 

 

After that, we can start other steps, as chemoprophylaxis.
