If this numbers are, indeed, correct, this is a case of multidrug resistance. Mouse foot pads assay must be done, as well as molecular biological evaluation on biopsy specimens.
Some points should be more clear, however: what is "normal bacilli"? Even in a lepromatous patient without treatment, most bacilli released from the globi are not viable, i.e., are fragmented. This occurs because the metabolism of M.leprae is too weak, and most bacteria die in vivo, even before the administration of antibiotics. We should not count the globi when evaluating MI.
