Dear Dr. Schreuder,

 

Regarding the diagnosis of leprosy without skin lesions (PK Das, LML, 14 June, 2015). We do see cases of primary or pure neuritic leprosy who do not develop skin lesions and where it very difficult to decide when to stop treatment. I will be grateful if u could inform about these markers that help to decide about this important decision.

 

Most of the times these patients come with deformities and sensory loss which generally does not improve with MDT. Some of these come back with new onset NFI a few months to years after RFT which could be relapse or late reversal reaction and it is both a diagnostic and management dilemma.

 

If we have markers which can help in assessing response that would be helpful both in these patients and others with NFI or if we plan some studies on some neuro-protective agents which can be given to leprosy patients.
