Dear Pieter,

 

I  agree with Dr. Marivic Balagon from Cebu regarding serious problems with migration and leprosy in urban areas of developing countries. Labour migration often leads to incomplete treatment of  active cases because  of their early  return back to native place after completion of the project work in cities but before completion of MDT course.  These migrant population stays in the slum areas of the cities for a limited number of months. Sometimes they leave for home during harvesting and festival seasons which interfere with  the regularity of DRUG INTAKE.

 

We take the following measures in those cases:

1. Active surveillance of these slum areas at least once a year to look for active cases.

2. Provision of contact card to migrants to help in completion of MDT at their native place after his/her return at the nearest drug distribution centre (Primary health Centre). This is possible in all states where the National Control/ Eradication  Program has been merged with General Health Services.
3. Provision of MDT for few months at a time to those migrant workers who visit their native place intermittently for harvest or festival seasons. In India we follow  these guidelines in our NLEP.
